This section provides the official forms required for managing various aspects of a PhD student’s academic path—from initial enrollment to requests for authorizations and final thesis submission. All documents are updated in accordance with current regulations and internal program procedures.
Purpose of the Forms
During the PhD program, students may be required to submit forms for:
- Declarations and self-certifications
- Requests for authorization (e.g., mobility, suspension, extension)
- Final thesis submission and application for thesis defense
- Changes in personal data or administrative details
Each form includes clear instructions and a list of required supporting documents.
Accessing the Forms
All official forms are available in a dedicated folder regularly updated by the PhD Office.
